An OptumHealth Bank HSA provides employees with the flexibility and freedom to maximize their health care dollars and take control of their health care spending. At the same time, they may:
• Enjoy tax benefits since contributions, interest earned and
eligible withdrawals may be tax-free.
• Own the funds in their account and be able to use the account
with another employer should they wish.
• Carry over their unused funds from year to year since there is no
“use-it-or-lose-it” provision.

Employer Notification Form (ENF)
Used during implementation to gather information from the employer about their requirements for a Health Savings Account (HSA). Brokers/Employers must submit ENF prior to Employee enrollment. This form can also be submitted to advise OptumHealth Financial Services of a change to the group (e.g., contact information, etc.). Broker information is also captured on the ENF so that brokers eligible to receive commissions are paid correctly.

Application must be completed, signed and returned to OptumHealth Bank to enroll in an HSA. Starred fields (*) are required.
