Provider Enrollment

Enrollment is easy.

Get the benefits of electronic claims settlement -- at no charge to you. If you are not yet enrolled into the OptumHealth^SM Electronic Payments and Statements network, you will be notified as your payers come onto the system. You will be given options for enrollment, including

• Online
  
• By Mail
  
• By Fax
  

Enrollment takes only a few minutes. Simply supply your contact and banking information, then select to receive your electronic remittance advice (ERA) through your clearinghouse or directly from the Electronic Payments and Statements site.

That’s it. Beginning with the first deposit through Electronic Payments and Statements, all payment and remittance information can be accessed online through the comprehensive user Web site, where you can view, print, download, and search information.

To enroll in EPS by mail or fax, download the enrollment form, complete it, and return it. You will be plugged in and saving money in no time.
